After your first consultation, you can view a digital summary of your Health Plan on the HealthHub app.
Follow these steps to view your Health Plan in HealthHub.
Step 1: Go to Healthier SG
Tap the Healthier SG banner on the homepage.
Step 2: Open your Health Plan
Scroll down and tap on Health Plan to review your discussion and health goals. To view past Health Plans, tap View Previous Health Plan.
Don't have a smartphone?
You can request a printed copy of your Health Plan from your clinic.
