- Singapore General Hospital
Specialty Centres
- National Cancer Centre Singapore
- National Dental Centre Singapore
- National Heart Centre Singapore
- Singapore National Eye Centre
Important notes:
- You can only request reports from institutions where you have received treatment or medical services.
- If your healthcare provider is not listed above, you will need to request medical reports directly from the institution.
- If you are requesting for a medical report that is not available via HealthHub, please send in your request to the respective institutions.
- If applying on behalf of your child or parent, add them as your care recipient on HealthHub first.
- Fees may apply - please check the respective healthcare institution's website for current charges.
- The list of participating institutions may be updated from time to time.
