How do I submit a request for medical reports/ certificates on HealthHub? What types of medical reports/ certificates can I request for?

Follow these steps to submit a request for medical reports/ certificates:

Step 1: Select Medical reports/ certificates requests
Log in to your HealthHub app and tap Services > Health records > Medical reports/ certificates requests.

Step 2: Select your healthcare institution
Tap Make new request and select your healthcare institution. You can choose the institution by entering a keyword or select from the list.

Step 3: Read and accept the Terms and Conditions
You can refer to the description notes tagged to each institution for the list of medical reports that are available. Make sure that you read and accept the Terms and Conditions. Tap Next to proceed.

Step 4: Fill up the details when prompted
Select your Admission/ Visit Date, Clinical Department, Report Type and Purpose of Request from the drop-down list. Upload any supporting document as single file if applicable and tap Next.

Step 5: Enter your particulars and contact details
Follow the on-screen instructions to input your name, contact number and email address.

Step 6: Select collection mode
Choose how you would like to receive the report. You can choose to either receive the report via registered mail or download it on HealthHub.

Step 7: Review your request and make payment
Ensure all details entered are correct, then tap Proceed to Payment. HealthHub accepts credit cards (Visa, Mastercard) and debit cards. Enter your card details when prompted.

Step 8: Verify your payment
You will need to verify and approve your payment through your banking app. Follow the on-screen instructions to launch your banking app and approve the transaction using the push notification or digital token within the given timeframe.

Step 9: Confirmation
Once your payment is successful, you will be notified confirming the transaction is complete. You may view the status of your requested report under the transaction history. Your report will be delivered using the collection method you selected once it is ready.


Important notes:

  • If your healthcare provider is not listed on HealthHub, you will need to request medical reports directly from the institution.
  • You can only request reports from institutions where you have received treatment or medical services.
  • Fees may apply - please check the respective healthcare institution's website for current charges.
  • The list of participating institutions may be updated from time to time.
