Who can use HealthHub to request medical reports?

Requesting for yourself:

  • You must be 21 years old and above

Requesting for others:

  • You must be 21 years old and above AND be a Legally Authorised Representative of the patient
  • To request medical reports for your care recipients through HealthHub, you must first ensure that you have added them as your care recipient in HealthHub. Once this is set up, you can proceed with the request.
  • You will need to provide photocopies of relevant documents (e.g. birth certificate, marriage certificate, letters of administration) as proof of your relationship to the patient, where applicable 

Important: 
If the patient is below 21 years old, deceased, or lacks mental capacity, you cannot use HealthHub for their medical reports. Instead, make the request directly with the relevant hospital.
