Overview
HealthHub gives you convenient access to your health records from participating public and private healthcare institutions across Singapore, held securely in various National Health Systems.
You can also request medical reports and certificates from selected public healthcare institutions, so you can keep track of your health and share key information with your healthcare providers.
Available record types include:

Immunisation records
What you can see:
- Your completed vaccination records based on the National Childhood Immunisation Schedule (NCIS) and National Adult Immunisation Schedule (NAIS)
- COVID-19 vaccination records as well as any other immunisation that you have completed

- Records come from National Immunisation Registry (NIR) and National Electronic Health Records (NEHR).
- Your records may not be comprehensive and may contain duplicates.

Health screening
What you can see:
Results from the health screenings done under Healthier SG Screening programme. The following health screenings are available under Healthier SG Screening:

Important
- Records come from Health Promotion Board (HPB).
- Records may take 2-3 weeks to appear after your screening and only show after you receive the hard copy.
- The tests have limitations and are not conclusive for detecting or ruling out medical conditions. Always consult a doctor if results are abnormal or if you feel unwell.
Medical reports / certificates
You can request medical reports from public healthcare institutions in Singapore. These include memos, Ordinary and Specialist Medical Reports, and duplicate copies of discharge summaries and investigation results. The report types available depend on where you received treatment.
Eligibility to use this service
Requesting for yourself:
- You must be 21 years old or above
Requesting for others:
- You must be 21 years old or above and be a Legally Authorised Representative of the patient
- You may need to provide photocopies of relevant documents (for example, birth certificate, marriage certificate or letters of administration) as proof of your relationship to the patient
Important
If the patient is below 21 years old, deceased, or lacks mental capacity, you cannot use HealthHub for their medical records. Please make the request directly with the relevant hospital.
Step 1
Log in to your HealthHub app, and tap Services > Health records > Medical reports / certificates requests
Step 2
Tap Make new request.
Step 3
Choose your healthcare institution by entering a keyword or selecting from the list.
Step 4
Read the description notes for available medical reports. Accept the Terms and Conditions and tap Next.
Step 5
Select your Admission/Visit Date, Clinical Department, Report Type, and Purpose of Request.
Upload supporting documents if needed and tap Next.
Step 6
Input your name, contact number, and email address as prompted.
Step 7
Select either registered mail delivery or download from HealthHub.
(Options to receive your report will differ by institution)
Step 8
Check all details are correct and tap Proceed to Payment.
HealthHub accepts Visa, Mastercard, and debit cards.
Step 9
Approve the transaction through your banking app.
Use push notification or digital token within the given timeframe.
Step 10
You'll receive confirmation once payment is successful.
Check transaction history for status updates. Your report will be delivered using your selected method.

The list of participating institutions may be updated from time to time. If your healthcare provider is not listed, you will need to request reports directly from that institution.
Types of Medical Reports Available
The reports available depend on where you were treated. When you select your healthcare provider during the application, you'll see the full list of reports that institution offers.

Common options include:
Medical Report/ Memo

Memo:
A statement from a doctor stating the patient's diagnosis, without an explanation of the condition. -
Ordinary Report:
A report prepared by the doctor based on the patient's medical records. It is a factual record of the patient's medical condition. -
Specialist Report:
Prepared by the patient's specialist when a professional opinion is needed on the patient's condition, prognosis or disabilities (for example, for an insurance form or a typewritten report). It is based on an actual assessment and may involve a review at the Specialist Outpatient Clinic.
Filling Up of Insurance Form

Simple Form:
An insurance form usually requested by the insurance company. It requires information such as the diagnosis, diagnosis code, procedure performed, procedure code and name of the referring doctor. -
Ordinary Claim:
A detailed insurance claim form completed by the doctor. It requires information such as the diagnosis, details of injuries suffered and treatment provided. -
Specialist/ Disability Claim:
A detailed insurance claim form provided by the insurance company for the doctor to assess the patient's disability or severity. It requires information such as the prognosis, diagnosis, details of injuries suffered and treatment given.
*Consultation fees will be charged separately by the clinic on the day of the assessment.
Others
Issuance of an LPA certificate. LPA report fees do not include consultation fees if the patient needs to be assessed by a specialist first.

A report prepared by the patient's psychiatrist when a professional opinion is needed on the patient's prognosis and disabilities. It is based on an actual assessment and may involve a review at the Psychological Medicine Specialist Outpatient Clinic. You will need to make an appointment with the clinic for the affidavit to be signed together with the Commissioner for Oaths.
Duplication of Documents

Discharge Summary:
A duplicate of an inpatient discharge summary, covering the patient's medical condition, diagnosis, procedures and medication given during a hospital stay.
*Refunds are not available if you cancel these report types. -
Investigation Results:
Duplicates of investigation results such as X-ray reports, CT scan reports, blood test results, ECG reports, histopathology reports, cytogenetic reports, bone density reports and urine test results.
*Refunds are not available if you cancel these report types.
Essential Reminders
- You can only request reports from institutions where you have received treatment or medical services
- If applying on behalf of your child or parent, add them as your care recipient on HealthHub first
- Fees apply, so check the institution's website for current charges
Your medical report is usually ready within 4 to 6 weeks of submitting your complete application, supporting documents and payment. Duplicate copies of investigation results, discharge summaries or medical certificates usually take 1 to 2 weeks.
Processing will only begin once the institution receives your completed application form, supporting documents and payment.
Processing may take longer if:
- The patient misses assessments needed to complete the report
- The patient has upcoming appointments or is currently warded
- The doctor assigned to the report is unavailable
- Reports are requested from several clinical departments
- Access to your medical information is blocked due to unforeseen circumstances
- The application form is incomplete, or the supporting documents needed for verification are not provided
- HealthHub accepts payment made using Visa, Mastercard, debit cards and eNETS (for supported institutions)
- Medisave cannot be used to pay for medical reports
- Fees cover administrative costs for reviewing your medical records and preparing the report
You can download completed reports from HealthHub, or have them posted to your local mailing address by registered mail. Postage fees may apply.
Contact the healthcare institution directly if:
- You are not a Singpass user
- You are applying for someone who cannot provide consent
- The medical report you need is not available on HealthHub
- You have payment enquiries
- You need clarification about a previous request
For NUHS institutions (AH, JCH, JMC, NUH, NTFGH)
- No refunds are available for memos, investigation results, medical report certifications, duplicate medical certificates, and discharge summaries
- Other reports may be cancelled within one calendar day of payment; thereafter, a $30 administrative fee applies
- No cancellation permitted once the medical officer has completed the report
Important
- Read the terms and conditions and cancellation policy carefully before making your application.
- If you have not received your report after 6 weeks, contact the institution directly to check on your request status.
- For SingHealth institutions, please verify cancellation policies directly with the healthcare institution.
Note
KK Women's and Children's Hospital sends reports by email by default. If you need original hard copies, arrange collection directly with the hospital. Refer to the institution's website for more information.

Medical Alert & Adverse Drug Reaction / Drug Allergy

- Records come from Critical Medical Information System (CMIS).
- If you no longer have G6PD Deficiency, the status will show as "NO" but the line item remains.

Medical certificates (MC) are currently not available through HealthHub. If your medical certificate was issued by public healthcare institutions under the NHG Health Group (NHG) and National University Health System (NUHS) healthcare clusters, you may view your medical certificates using the NHG Health App and NUHS App respectively, under the Test Results & MC section.
The cost depends on the type of medical report and the institution that treated you. For the latest fees and charges, refer to your institution's website.
A medical report is a detailed summary of the patient's medical condition, and the doctor needs to review the patient's medical records to produce an accurate report. Medical reports are not covered by regular consultation and hospitalisation fees. The fee covers the hospital's administrative cost of producing the report. This is standard practice across all public healthcare institutions in Singapore.
